You will see a question, “Are you a CareMount patient?” The answer “yes” will be filled-in for you. Click “Continue“.
Enter your email and create a user name and password (which must be 8 characters long).
If this email address is being used by another person in your home, you will get a message stating so. Click “Continue“.
(REQUIRED) The next screen requires you to select 2 security questions. Click “Next” when done.
On this next screen enter your address (City/State/Zip).
This next screen asks for your phone number, which is optional.
Next enter your weight and height, gender, preferred language and if you require an impaired hearing service.
Next you will be asked about your medical history. THE ONLY REQUIRED STEP IS TO FILL OUT THE SECTION LABELED SOCIAL HISTORY, WHICH IS VERY IMPORTANT.
Follow the steps to complete your registration. Once you are registered, you will receive a “welcome” email from CareMount 24/7. Then it is time to schedule your visit by calling or scheduling online.
You must call 1-833-525-0221 (Mon-Fri 8-6 and Sat 8-5) or book online to schedule a visit. Once you have booked an appointment with a CareMount provider, your scheduled visit will appear on the screen.
Log into CareMount 24/7 on your computer 15 minutes prior to your appointment.
Click “Connect” to connect to your scheduled visit.
When asked: “Where will the visit take place?” Select “New York“.
Click “VIDEO” under “How would you like to connect?”. Click “CONTINUE“.
