CareMount Medical has been recognized by the National Committee for Quality Assurance (NCQA) as a Level 3 Patient-Centered Medical Home (PCMH). A PCMH is a system of care designed to provide better patient care, an improved patient experience, continuity of care, and disease prevention and management. The term medical ‘home’ was designed to convey a feeling of being at home with your medical care.
Your primary care physician (PCP) leads the PCMH team, which includes specialized physicians, nurses, health educators, and other health care professionals. This group works together to create a long-term partnership with you to manage all of your health care needs. Here is what a PCMH does for you.
PCMH always puts the patient front and center. Healthcare decisions are made with your active involvement and is based on your individual preferences.
Everyone at the practice, from physicians to front desk staff, work as a team to coordinate care from other providers and community resources. This works to avoid preventable complications, hospital admissions, and emergencies by focusing on prevention and managing chronic diseases.
We really get to know you. We have access to your personal and family health history, your life situation, and your preferences so we can make recommendations that make sense for you.
We’ll help you understand your conditions and the best way to take care of yourself. We’ll make sure you understand your options and help you when making health care decisions.
Through electronic medical record keeping, we are able to quickly and securely share your medical records with other team members. For instance, when you get a lab test, everyone on the team will have access to the results. The same holds true if you visit one of our Urgent Care centers. This ensures that your care is always coordinated and helps prevent potential medical errors.
The bottom line is that we’re there for you every step of the way. We’ll help you set healthcare goals and keep you on track. We’ll also provide information about classes, support groups, seminars, or any other services that can help you learn more about a particular condition – or to just stay healthy. You are the most important part of a patient-centered medical home and we’ll make sure you feel that way.
Patients who have a limited insurance policy or no insurance can reach out to our Patient Assistance team for financial guidance. Our Patient Assistance Representatives can provide information about Managed Medicaid, other NY State insurance options, payment plans, and our financial hardship program.
