NCQA Patient-Centered Medical Home™ Standards Emphasize Enhanced Care Through Patient-Clinician Partnership

Chappaqua, New York—February 22, 2018—CareMount Medical, the largest independent multispecialty medical group in New York State, has been recognized again by the National Committee for Quality Assurance (NCQA) as a Level 3 Patient-Centered Medical Home (PCMH).   PCMH is a system of care that uses evidence-based, patient-centered processes, focusing on highly coordinated care and long-term relationship building. CareMount Medical earned a Level 3 PCMH Recognition due to its commitment to the patient’s experience.

All of CareMount Medical’s 19 Primary Care locations received recognition as Level 3 Patient-Centered Medical Homes in December 2017.  CareMount Medical has had the third highest number of facilities designated with a Level 3 designation in the U.S.
