CareMount Medical joins UnitedHealthcare’s Medicare Advantage network through accountable care relationship

  • UnitedHealthcare Medicare Advantage enrollees now have access to more than 600 CareMount Medical primary care physicians, specialists and advanced practice professionals.

CHAPPAQUA, N.Y. (Sept. 23, 2019) – CareMount Medical and UnitedHealthcare have established a new network relationship, providing people enrolled in UnitedHealthcare Medicare Advantage plans access to more than 600 CareMount Medical primary care physicians, specialists and advanced practice professionals throughout the Hudson Valley. The organizations are also launching an accountable care program that will encourage better health, better care and a better experience for CareMount patients enrolled in UnitedHealthcare Medicare Advantage plans.

Through the new accountable care relationship, UnitedHealthcare will provide an enhanced level of data and support to CareMount to enable its providers to deliver more comprehensive care to their patients. For example, UnitedHealthcare will share data about its members’ underlying medical conditions, past treatments, gaps in care, medications prescribed and future care needs.

The data will be actionable and specific to individual patients, giving CareMount physicians a fuller picture of their patients’ health status and arming them with valuable information that supports their efforts to provide quality care. For example, physicians can more easily identify patients who are at higher risk for an adverse health outcome such as an emergency room visit or readmission to the hospital, allowing them to intervene more quickly. They can also use the data to reduce duplicative tests and help patients manage their chronic conditions and take their medications as prescribed.

Ultimately the accountable care program will improve care coordination across specialties and care settings. This in turn can lead to a better experience for UnitedHealthcare Medicare Advantage members and CareMount patients and reduce the burden of having to connect information from each of their doctor visits themselves.

The accountable care relationship with UnitedHealthcare is one of many risk-based payment models that CareMount participates in to help lead to improved health outcomes and lower costs for its patients. Currently, CareMount serves approximately 40,000 Medicare patients through risk-based contracts. The new relationship with UnitedHealthcare allows CareMount to expand patient choice.
