CareMount is One of 17 New CMS Next Generation Accountable Care Organizations in U.S.A.
Chappaqua, New York—April 24, 2018—CareMount Medical, the largest independent multi-specialty medical group in New York State, has joined the Centers for Medicare & Medicaid Services’s (CMS) Next Generation Accountable Care Organization (ACO) Model under its managed care subsidiary, CareMount ACO. The ACO will focus on risk-based contracts for Government Programs, including Medicare Advantage. Next Generation ACOs are comprised of groups of doctors, hospitals, and other healthcare providers who come together voluntarily to deliver coordinated, high-quality care to improve the patient’s experience and health while lowering overall healthcare costs. CareMount Medical participated as a Medicare Shared Savings Program Track 1 ACO since 2012 before advancing to the Next Generation ACO Model in 2018.

Patients aligned to the Next Generation ACO Model will see no change to their original Medicare benefits and will continue to have the option to see their choice of Medicare provider. Patients receiving services through CareMount Medical’s ACO have received a letter explaining the new model of care. Patients not currently receiving primary care services through CareMount ACO could be eligible to be included in the ACO in the future.
Under its value-based care model, CareMount ACO will deliver care coordination services in close collaboration with more than 560 CareMount Medical primary care physicians, specialists and advanced practice professionals. The ACO physicians will work collaboratively to ensure their patients, especially those with chronic conditions, receive the highest value services, at the right time and setting. Patient engagement is a key part of the process, and ACO physicians will partner with patients and their families in decision making. Further, enhanced patient protections guarantee patients have access to and receive high-quality care.
CMS assesses Next Generation ACOs annually on their overall quality of care measures and ability to deliver high-value care outcomes to attributed beneficiaries. CareMount ACO will provide performance measures on its ACO website in 2019.
