Agreement improves care coordination, enhances referral process for hospice and palliative care patients
Chappaqua, New York—February 10, 2020— CareMount Medical, the largest independent multi-specialty medical group in New York State, today announced that it has entered into a Preferred Provider Agreement with Hudson Valley Hospice, a nonprofit community agency that delivers compassionate care to individuals faced with life-limiting illnesses.

As part of the agreement, which took effect on January 1, the two organizations have implemented enhanced referral and communications policies to drive better care coordination for CareMount patients receiving palliative and hospice care. By creating better alignment between CareMount and Hudson Valley Hospice, the new venture simplifies and streamlines logistics for patients and their families, freeing them to focus on obtaining the care they need.
CareMount’s Preferred Provider Agreement with Hudson Valley Hospice is part of the medical group’s broader strategy to fully integrate care delivery around its independent physicians. Since launching its Medicare Next Generation Accountable Care Organization (Next Generation ACO) in 2018, CareMount has added preferred providers across the acute inpatient hospital, skilled nursing facility and home care settings. These preferred providers coordinate care for CareMount primary care patients under Medicare and commercial payer risk contracts and partner with CareMount to improve quality and clinical-care outcomes. CareMount Health Solutions, CareMount’s management services organization, runs the only physician-led Next Generation ACO contract in the Tri-State area.
