CareMount Expands Into Advanced Risk-Based Contracts with Three Medicare Advantage Plans

Chappaqua, New YorkJuly 18, 2018CareMount Medical, the largest independent multi-specialty medical group in New York State, has expanded into risk-based partnerships with three leading Medicare Advantage payers, including Aetna, Anthem, and WellCare.

Medicare Advantage (MA) plans typically offer more benefits than traditional Medicare, including prescription drug coverage, dental, vision, and more.  Partnering with MA plans allows CareMount physicians to deliver other high-value services such as preventative services, hospital discharge planning, care coordination and outreach. CareMount physicians are considered in-network for these health plans, resulting in lower out-of-pocket costs for patients compared to seeing a healthcare provider outside of these plan networks.

“By partnering with these top plans, we are focusing on the importance of preventive and value-based care for our senior patients,” said Scott D. Hayworth, MD, FACOG, President and CEO of CareMount Medical.  “Our Medicare Advantage partnerships further underscore our commitment to providing CareMount seniors with outstanding care and access to programs and services that improve health and well-being – all with the comfort and convenience of being close to home.”

Under its value-based care model, more than 560 CareMount Medical primary care physicians, specialists and advanced practice professionals deliver clinical service and care coordination to seniors across the Hudson Valley region.  CareMount’s recent expansion into Medicare Advantage will empower this care model and aligns with the group’s participation in the Medicare Next Generation ACO program.   Currently CareMount serves approximately 40,000 Medicare seniors under advanced risk-based contracts.  Both Medicare segments – ACO and risk-based MA – support CareMount as it expands into Population Health and takes accountability for its patients’ total cost of care and quality outcomes.

Patients can learn more about these new MA plan options at local educational events hosted by CareMount Medical and held throughout the year.
